Can my spouse get insurance also?
Your spouse is eligible for any insurance you, as an IMRF member, are eligible for. (same with all programs)
Does it cost me or the IMRF extra to get insurance through you?
No.
Will my spouse be eligible for insurance, even if I am not taking the insurance yet?
Yes.
What is the sign up period for Insurance?
We have rolling enrollment for all insurance, with the exception of Medicare Part D, which has federally mandated enrollment periods.
When will my coverage become effective?

Vision and Dental Insurance: If we have your application and premium deduction authorization by the 25th of the month, then your effective date will be the 1st of the following month.

Medicare Part D Prescription Drug Plans: You are able to sign up during annual enrollment: Nov. 15 – Dec. 31; or within 60 days of the termination of your group health insurance (with creditable coverage); or within 60 days of Part B eligibility.

Supplemental: You may enroll in a supplement at any time, however, if you enroll within 90 days of your Medicare Part B eligibility or termination of another plan you will not be subject to any pre-existing conditions waiting periods.

Under 65 Medical Plans: You may apply at any time. (Approval is subject to medical underwriting.)
How long does it take to be approved? How far in advance do I need to apply?

Under 65 Blue Cross Blue Shield: 30 - 60 days. You cannot apply more than 60 days before effective date.

Supplemental: 1 week - 30 days.

Medicare Part D Prescription Drug Plans: May not apply any sooner than one month before requested effective date.
Will I get an ID card for Dental and Vision?
Dental: Yes
Vision: No
If I don’t get an ID card for Vision, how will they know I have insurance?
When you go to an in-network provider, tell them that you are with United HealthCare Vision, provide them with your Social Security Number and date of birth, and they will look up the enrollment information on the computer system.
How do I find out who the providers are for Dental and Vision?
For Dental, click here to visit Delta Dental IL. Enter your location information and select "Search for a Dentist."
For Vision, click here to visit United Healthcare Vision. Select "Future Member" and enter the location information requested. Click "Submit."
Can I fax the forms back to DRL?
Doyle Rowe LTD must have the original application in order to process the application.
How do I cancel my insurance?
All cancellations must be in writing. Send to: Doyle Rowe LTD, 1301 W. 22nd Street, Suite 101 Oak Brook, IL 60523. Note: You must remain in the dental and vision plans for one year.
Federal government rules must be followed for Medicare Part D dis-enrollments.
How do I sign up for dental and vision?
Both Vision and Dental plans require you to fill out a premium deduction authorization form and an enrollment/application form.
Do I have to have Dental and Vision deducted from my pension check?
Yes, if you are a retiree, your payment for Dental and Vision plans are deducted from the IMRF pension check.
Can I sign up for Delta Dental if I am an Active member?
Yes. Payments may be made via EFT (Electronic Funds Transfer), or with quarterly, semiannual or annual billing.
Can I get Dental and/or Vision without having other insurance with you?
Yes.
What happens when my spouse dies?

For the Delta Dental, UHC Vision, Blue Cross Blue Shield Supplemental plans: Need to send a copy of the Certificate of Death to Doyle Rowe LTD

For Delta Dental & UHC Vision: if deceased spouse is IMRF member, then your Delta Dental and UHC Vision will automatically be cancelled. If you would like to remain on the plan, please contact us.
IMRF Members Under 65
Are the Blue Cross Blue Shield Under 65 plans Group Plans?
No, they are individual plans.
Am I automatically accepted into the BCBS Under 65 plan?
No, you must fill out an application, which will go through the medical underwriting process to be approved or denied.
What does underwriting mean?
It is a review of your medical records.
How long does it take to be approved? How far in advance do I need to apply?
What if I am not accepted?
Then you will be issued a refund of the two months premium check that is submitted at the time of application.
What is the best plan?
It depends on your wants and needs.

What is an HSA?
An HSA is a Health Savings Account that is set up with your bank.
How far back do they consider a pre-existing condition?
12 months.
How do I get my money back if I am not accepted?
BCBS will issue a refund check.
Does the deductible apply to prescriptions?
It depends on the plan and deductible. For plans with prescription drugs covered at 80%, the 80% coverage starts after the plan deductible is met.
