• Available to Illinois and Florida residents age 65 and older with Medicare Parts A & B. In Network Benefits include the following:
  
• $10.00 physician visit co-pay
• $35.00 specialist visit co-pay
• $10/$35 preventive service co-pay
• $165.00 per day for inpatient hospital stay (days one through five)
• $100.00-$125.00 outpatient surgery co-pay
• $0.00-$75.00 outpatient non-surgical co-pay (depending on place of service)
• $3,000.00 annual out-of-pocket maximum
• Plan includes Medicare Part D prescription drug benefit: $5/$30/$60/25% co-pay; 90-day mail order 2x co-pay for levels 2 & 3. Once drug costs exceed $2,510.00, you pay $5.00 co-pay for generics and 100% for preferred, brand and specialty drugs. After total out-of-pocket costs exceed $4,050.00, you pay the greater of $2.25 for generics, $5.60 for preferred and brand name drugs, or 5% co-insurance. You pay 5% for specialty drugs.
  Most out of network services paid at 70% after $300 deductible.
