City of Chicago Laborer and Municipal Annuitant Retirees under age of 65 and/or their families who reside in Illinois are eligible to enroll in Blue Cross and Blue Shield of Illinois-Under age 65 plans. Various deductibles and plan design options are available.

NON-MEDICARE RETIREES

If you choose to enroll in an Individual HMO or PPO simply click on the “Select” button and complete the online application.

Open enrollment for individual plans runs from November 1, 2017 — December 15, 2017 for January 1, 2018 effective date.  Otherwise you must qualify for a Special Enrollment Period (SEP).  Loss of group coverage may qualify you for an SEP.

Do I Qualify For A Subsidy?

Some individuals may qualify for an Affordable Care Act subsidy to assist with premium costs.  Generally the guidelines are as follows:

Household Size                             2018 Projected Modified Adjusted Gross Income

1                                                                            Less than  $48,240.00

2                                                                            Less than  $64,960.00

3                                                                             Less than $81,680.00

4                                                                             Less than $98,400.00

How Do I Use My Plan?

If your primary concern is being covered should a catastrophic illness or injury occur you may want to select an individual plan with lower premiums and higher deductible and out of pocket maximums.  These plans often require you to pay for your doctor’s office visits and prescriptions until you meet your deductible.

If however, if you prefer lower cost sharing in the form of deductibles, co-pays an co-insurance amounts you will pay a higher premium and may prefer a group plan.

 

MEDICARE ELIGIBLE RETIREES

You also have the option of selection individual Medicare Advantage or Medicare Supplement plans along with a Medicare Part D prescription drug plan. These options may be more cost effective than the group option.  If you would like to explore these options please call 1-866-201-2524.
