Health Alliance HMO

The Health Alliance HMO is available to retirees and spouses age 65 and older with Medicare Parts A & B residing in the Illinois counties below:

 

Champaign, Coles, Ford, Douglas, Piatt, McLean, Moultrie, Woodford, Vermilion and DeWitt counties

 

In-Network Health Plan Costs:

• $20.00 doctor office visit co-pay

• $40.00 specialist office visit co-pay

• $100.00 per outpatient surgery co-pay

• $100.00 co-pay each day(s) 1-7, $0.00 co-pay 8 and beyond per hospital admission

• $0.00 co-pay for preventative and lab services

 

Prescription Drug Coverage:

• Plan includes a Medicare Part D prescription drug benefit (participant must use an in-network pharmacy).

• Co-pays are: $20.00 for Tier 1 Preferred generic ; $40.00 for Tier 2 Non-preferred generic; $40.00 for Tier 3 Preferred brand; $50.00 for Tier 4 Non-preferred brand; $60.00 for Tier 5 Specialty Pharmacy.

• Includes coverage through the coverage gap (“donut hole”) for all tiers.

• Catastrophic coverage (when out of pocket costs reach $4,950.00).

• Generics $3.30 or 5% whichever is higher.

• All other $8.25 of 5% whichever is higher.

• Once your total yearly drug costs reach $3,700.00, you pay 40% of plans cost for covered brand name drugs and 51% of plan’s cost for generic drugs until your costs total $4,950.00. Then you pay, greater of $3.30 for tier 1 or 2 and $8.25 for all other or 5% coinsurance.

The Health Alliance POS is available to retirees and spouses age 65 and older with Medicare Parts A & B residing in the Illinois counties below:

 

Adams, Brown, Bureau, Cass, Christian, Clark, Crawford, Cumberland, Edgar, Effingham, Hancock, Iroquois, Jasper, Livingston, Logan, Macoupin, Marshall, Mason, McDonough, Menard, Montgomery, Morgan, Peoria, Pike, Putnam, Sangamon, Schuyler, Scott, Stark, Tazewell, Champaign, Coles, DeWitt, Douglas, Ford, McLean, Moultrie, Piatt, Vermilion, and Woodford counties

 

In-Network Health Plan Costs:

• $20.00 doctor office visit co-pay

• $30.00 specialist office visit co-pay

• $0.00 annual exam co-pay

• $175.00 per outpatient surgery co-pay

• Inpatient hospital costs: days 1-10 $195 copay per day,  days 11 + $0 copay per day

• $0.00 co-pay for preventative and lab services

Underwritten by Guarantee Trust Life, these plans offer the freedom to choose doctors and hospitals.

2017 Monthly Medical Plan Rates (based on plan choice and age) $181.93 — $432.58

 

Prescription Drug Benefits

 

• Enrollees may also choose one of three Medicare prescription drug plans.  (You must also be enrolled in one of the three medical plans).

• Prescription drug plans have separate monthly premiums, based upon plan choice.

2017 Rx premiums range from $131.93 — $353.30

Humana Regional PPO

Available to retirees and spouses age 65 and older with Medicare Parts A & B in specific cities throughout the country based on network accessibility.

Plan highlights include:

• 100% of in-network preventive services including annual routine physical, diabetes self-management, bone mass measurement (one per year), immunizations,  colorectal screening (one per year), and the like.

• A variety of co-pays for physicians, specialists, and other medical services.

Prescription Drug Benefits:

Plan includes Medicare Part D prescription drug benefits. After total out-of-pocket costs exceed $4,950.00, you pay the greater of $3.30 for generics, $8.25 for preferred and brand name drugs, or 5% co-insurance. You pay 5% for specialty drugs.

United Healthcare Medicare Complete

United Healthcare Medicare Complete is available to retirees and spouses age 65 and older with Medicare Parts A & B residing in Monroe, Madison, Jersey and St. Clair counties in Illinois and St. Louis City, St. Louis County, St. Charles, Jefferson, Warren, Franklin, Crawford, Lincoln, Washington and Gasconade counties in Missouri.

In-Network Health Plan Summary

• 100% of specified preventive services including annual routine physical, diabetes self-management, bone mass measurement (one per year), immunizations, colorectal screening (one per year) and the like.

• A variety of co-pays for physicians, specialists and other medical services.
